Guilford County Sheriff's Department

*It's plain to see J.D. Church
[unreadable] he writes them to say what he wants them to say.
APatricia took care of
this (after we argued)
BWhy would she just ask?
Page 203 shows her teeth and their great. Patricia thinks
insurance is a waist of money (It must have been me)
C9/11/95 We need a copy of
cancer & dental policys (I don't remember buying Dental)
DI didn't call his house.
He was pushing insurance on 9/11/95 and was going to run some quotes
and return on 9/12/95. How many insurance salesmen only try to
sale you one policy?
EJerrell quoted me on
several amounts of insurance.
FNotice "that day," "next
day". In the statement a year before it was a different story.
See page 166 (e). He did keep coming trying to sale me
insurance.
GTrue
HSaid something similar
but can't recall. The call wasn't in this order.
INever happed. She came by
and dropped off lunch and left. Jerrell left the application
with me to have her sign it.
JIt was 150,000 plus the
25,000
KHe said the information
he went to the car for was on the cancer policy, see page 167 (A)
LI'm not sure he even went
to the car. I believe Patricia brought lunch in and set it
down. Stated she didn't have time to talk about it, she had to
go and she left.
MHe claims she drove off
but a year earlier he didn't state this.
NHow did he find this out?
See page 167 (N) (Front) He lives in Winston Salem. I called
and told him that I signed it and he should rewrite the policy and
let her sign it. He said it would be O.K. when she went for
the physical everything would work out. He was afraid of
loosing the sale.

CASH FOR THE LIFE INSURANCE THAT WAS
BEING APPLIED FOR.
JARRELL SAID THAT TED CALLED HIM
SEVERAL TIMES AFTER THIS TELLING HIM THAT THE COMPANY WOULD NOT PAY
BECAUSE HE WAS A SUSPECT AND WANTING JARRELL TO HELP HIM IN GETTING
THE MONEY FROM THE TWO $25,000.00 POLICIES. HE TOLD JARRELL THAT HE
WAS IN FACT A SUSPECT. JARRELLH
TOLD HIM THAT HE WOULD HAVE TO CONTACT THE MAIN OFFICE THAT HE HAD
NOTHING TO DO WITH WHETHER THE COMPANY PAID OR DIDN'T PAY. TED ALSO
TOLD JARRELL THAT PATRICIA ALSO HAD A $25,000.00 POLICY AT HER WORK
BUT THAT HER MOTHER WAS THE BENIFICIARY AND THAT HE HAD ASKED HER
MOTHER FOR THE MONEYI
AND THAT SHE HAD TOLD HIM THAT SHE WOULD GIVE IT TO HIM. IN ONE OF
THE PHONE CALLS AFTER PATRICIAS DEATH TED ASKED JARRELL IF THE
POLICY WOULD PAY DOUBLE, OR WAS DOUBLE INDEMENTY AND TED WAS TOLD,
NO.J
JARRELL SAID THAT TED HAD CALLED HIM AS LATE AS THE FIRST WEEK IN
AUGUST 1996 ABOUT THE INSURANCE POLICIES. HE ALSO SAID THAT HE HAD
GOT A PACK OF PAPERS FROM TEDS ATTORNEY ABOUT A MONTH AGO AND WHEN
HE REALIZED WHATK
THEY WERE THAT HE SENT THEM ON TO THE HOME OFFICE IN ATLANTA. TED
HAD ASKED DURING THE APPLICATION THAT IF HE GOT KILLED IN AN
AUTOMOBILE ACCIDENT WOULD THE INSURANCE PAY AND JARRELL TOLD HIM
THATL
THE APPLICATION PROCESS HAD TO BE
COMPLETE AND APPROVED AND THEN THEY WOULD PAY.

AOn the 1st statement he
said she replied "Oh, Ok." p 167 H.
BI thault I paid for a
cancer policy for her also but I've never seen it.
CPatricia called me about
it and I said we would talk about it when we got home. Jerrell
called & I told him the same. Patricia and I argued about it
that evening. (Se acted the same way back in March, she thault
it was a waist of money). Over the following 1 or 2 days we
talked it out and she agreed. I called Jerrel and told him I
signed the policy. I told him he could call Patricia and clear
everything with her. *See page 169 (N). He never called
Patricia as far as I know. Port-a-medic calls me at work
wanting to get us in. They were as pushy as the salesman.
DP.F.G. had me on First
Shift see page 167 (J)
EJD Church wrote this
statement. Jerrell didn't say this a year earlier. See
p. 168 (A)
FThe way this is worded I
never ask about the 2 x 25,000 policy when I first called, I only
ask about the 200,000.
GI did sign a claim.
Why did the insurance company say I never did?
HHe told me to get a
lawyer.
II didn't ask Sheila for
the money. I told her that she was the beneficiary and
that Patricia had forgotten to change it. She said, Don't
worry about it I'll give it to you. "The following week that
changed."
JHe didn't say this on
page 168
KIf I had a lawyer why was
I calling him? I may have inquired about my refund. I
believe I called the main office asking for my money but not about
the policys
LWe were speaking of the
single 50,000 policy on me. The agent can approve up to
$99,000 on the spot without the physical. From the moment I
paid him I was covered on this policy.
MPage 167(I) He claims I
called 10 times, but on this page he claims I never called.
