3. If something is
wrong on the Medical Board Cover Sheet, please let me know what is
wrong and I will make the corrections. 4. If there is
something in the dictation that you disagree with you have an
opportunity to submit a statement in rebuttal. If you rebut, you
have 5 working days to submit your rebuttal. 5. On the last
paragraph of the dictation, if your are not submitting a rebuttal -
you will need to circle the words "does not" and place your initials
under the circle. On the NAVMED 6100/2, the statement that starts
with "Having been informed of the contents, ... - you will need to
circle the words "do not" and initial. 6. If you are
submitting a rebuttal - you will need to circle the word "does" and
place your initials under the circle. On the NAVMED 6100/2, you will
need to circle the word "do" and initial. 7. Sign your name
and give current unit address on the NAVMED 6100/2. 8. The last
statement on the page that reads: "I certify that I am/am not
processing for separation/retirement" - you will need to circle the
words, "am not" and "separation" and initial both circles, if you
are not processing for an Administrative Separation. 9. If you are processing for an Administrative Separation, you will need to circle the words, "am" and "separation" and initial both circles. 10. Please
have your attorney witness and date the paperwork. 11. Please
return the paperwork to me within two to three working days after

Note: If you are submitting a rebuttal to the 13. This is
not your only opportunity to rebut, you can rebut the Preliminary
Findings when they return from. It normally takes four to eight
weeks from the mailing date to get the findings back. 14. The
medical board package must have a copy of the physical exam (SF 88,
SF 93, DD Form 2697, and Lab Results) as part of its package before
